Job summary

GEM Transition Care Center in Pasadena, CA, seeks a licensed Physical Therapy Assistant to join the rehabilitation team. You will collaborate with therapists, patients, families, and the IDT to guide personalized therapy plans and help residents regain independence.

Responsibilities include administering treatments, monitoring responses, and documenting progress while adhering to supervising therapist instructions.

Qualifications

• CPR certification is mandatory.
• Licensed Physical Therapy Assistant (PTA) required.
• Prefer 1–2 years of long-term care experience.
• Must be in good physical and mental health.
• Capable of lifting up to 40 pounds unassisted.
• Proficient in following oral and written instructions.
• Training/experience for supervising responsibilities.

Responsibilities

• Fundamental understanding of physical medicine, PT modalities, anatomy, and physiology.
• Basic knowledge of operating, maintaining, and repairing PT machines.
• Administers treatment and rehabilitation services to patients with injuries or disabilities. Conducts exercises with residents.
• Follows treatment instructions from supervising Physical Therapist. Adheres to physical medicine practice standards.
• Evaluates treatment effects, discusses observations with staff and Physical Therapist.
• Monitors and documents resident treatment and responses.
• Contributes to creating a teaching plan based on identified learning needs, assesses teaching effectiveness, involves family when necessary, and informs staff and Physical Therapist of outcomes.

GEM Transition Care Center is a 75 bed skilled nursing facility specializing in short-term therapy and dedicated to providing all of our residents with the type of care they deserve.

We offer a variety of therapies and care services to assist patients in regaining their independence, whether they are recovering from a hospital stay or require ongoing medical support. As the Physical Therapy Assistant and a valued team member, you will collaborate with therapists, patients, their families and the IDT to guide each patient through a personalized therapy program, helping them reach their recovery goals.

REQUIREMENTS:

1. CPR Certification (mandatory)
2. Licensed Physical Therapy Assistant (required)
3. Preferably 1-2 years of experience in long-term care.
4. Good physical and mental health are required.
5. Capable of meeting physical demands of the job, including lifting up to 40 pounds unassisted.
6. Proficient in following oral and written instructions.
7. Possess training, knowledge, and experience for general supervision of responsibilities.

REPORTING:

The Physical Therapist Assistant reports to the Rehabilitation Director.
