Job summary

Creative Solutions in Healthcare, Inc. is seeking a full-time Physical Therapy Assistant (PTA) to join an in-house rehab team. The PTA will deliver person-centered care to improve mobility, strength, and quality of life for residents with supportive supervision from a Physical Therapist.

Qualified candidates hold an active Texas PTA license in good standing, with SNF or long-term care exposure preferred. Familiarity with Net Health EHR is a plus, and strong communication is essential for team

Qualifications

• Active and valid Texas PTA license.
• In good standing with regulatory agencies and boards.
• SNF or long-term care experience preferred.
• Knowledge of physical therapy treatment practices.
• Experience with Net Health EHR preferred.
• Ability to manage patients with varying needs; strong teamwork.

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function.
• Provide skilled PT services under supervision of a Physical Therapist.
• Relay information and referrals to the PT upon requests.
• Implement and adjust care plans under PT supervision.
• Measure and record motor function, strength, and muscle performance.
• Monitor patient responses and adapt approaches with the interdisciplinary team.
• Educate patients, families, and caregivers clinically.
• Ensure timely and thorough documentation per policies and regulations.
• Attend meetings as designated by the Director of Rehab.

Join Our Team as a Full-Time Physical Therapy Assistant (PTA)

We are seeking a compassionate and motivated Physical Therapy Assistant (PTA) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapy Assistant (PTA)

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing

• Provide skilled physical therapy services and interventions in accordance with physician orders under the supervision of a Physical Therapist

• Respond to requests for service by relaying information and referrals to the Physical Therapist

• Implement and modify patients' physical therapy plans of care under PT supervision

• Measure and record patients' motor function, strength, and muscle performance

• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team

• Provide clinical support and education to patients, family members, and caregivers

• Ensure documentation is completed thoroughly and timely in compliance with company policies and state/federal regulations

• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Physical Therapy Assistant license

• Skilled Nursing Facility (SNF) or long-term care experience preferred

• Current knowledge of physical therapy treatment practices

• Experience with Net Health Electronic Documentation System preferred
