Join our team that delivers care with Meraki! We are seeking a per-diem PTA (Physical Therapy Assistant).

  • Provides therapy services as established by the Physical Therapist (PT) in the patient Plan of Care.
  • Provides therapy services under the supervision of a PT, in compliance with State and Federal guidelines.
  • Consults with PT regarding changes in treatment or patient condition in a timely manner.
  • Completes documentation including daily notes, progress notes, discharge summaries, or monthly reports as needed.
  • Submits billing, payroll, and expense documentation promptly.
  • Communicates effectively with patients, families, referring physicians, and team members.
  • Attends continuing education seminars to stay updated on clinical skills and techniques.
  • Participates in care planning, rehab meetings, and family training to communicate patient progress.
  • Follows organizational policies to ensure safety for employees and patients.
  • Ensures equipment is in good working order and reports damaged equipment.
  • Maintains a clean environment.
  • Attends meetings, in-services, and functions as required.
  • Coordinates treatment goals with team members.
  • Maintains valid licensure, certification, or equivalent as required by the state.

Requirements:

  • Associate of Arts in Physical Therapy Assistant or equivalent.

Licensure/Certification:

  • Licensure or Certification as required by State guidelines.
  • CPR Certification.

Experience:

  • 1 year of Physical Therapy Assistant experience preferred.
  • Experience in residential, assisted living, or skilled nursing facilities preferred.
  • Graduates of Physical Therapy Assistant programs will be considered.
