Continuum Rehab Group in Albuquerque is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities. You will provide rehabilitative services under the supervision of a Physical Therapist and contribute to evidence-based interventions that support patients’ recovery and quality of life.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.

• Assesses and documents patient progress; communicates with supervising PT.
• Provides counseling to patients and families about physical disorders.
• Contributes to discharge planning per clinical requirements.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient's progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
