Position Summary:

We are seeking a reliable and experienced Physical Therapy Assistant (PTA) to join our team on a per diem basis. The PTA will provide rehabilitative services to patients in their homes under the direction and supervision of a licensed Physical Therapist. This role is ideal for a self-motivated professional who enjoys flexible scheduling and working independently in the field.

Primary Responsibilities:

• Provide physical therapy treatments to patients in accordance with the Plan of Care established by the supervising PT
• Monitor and document patient progress; report changes to the supervising PT
• Instruct patients and caregivers on home exercise programs and safety
• Collaborate with interdisciplinary team members to ensure coordinated care
• Maintain timely and accurate clinical documentation in the EMR
• Comply with Medicare, Medicaid, and agency standards and regulations
  
  
  

Qualifications:

• Active Florida license as a Physical Therapist Assistant (PTA)

• Minimum of 1 year of experience in home health or outpatient physical therapy preferred
• Familiarity with Medicare documentation requirements
• Reliable transportation, valid driver’s license, and current auto insurance
• Ability to work independently and manage time effectively
