Job summary

Discovery at Home is seeking a licensed Physical Therapy Assistant to support home health and outpatient services across Florida. You will assist the Physical Therapist, help patients with exercises and ADLs, and document progress as part of an interdisciplinary team.

Qualifications include graduation from an accredited PTA program, state licensure, CPR, valid driver’s license, and strong communication skills.

Qualifications

• Must be graduate of an accredited Physical Therapy Assistant program.
• Must be licensed by the State as a Physical Therapy Assistant.
• One year of home health experience preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency (Microsoft Windows).

Responsibilities

• Assist the Physical Therapist in planning and implementing patient care and therapy services.
• Assist patients with exercises, mobility, and ADLs as directed by the PT.
• Educate patients and caregivers on techniques to improve mobility and prevent injuries.
• Maintain accurate and timely documentation of patient progress.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Provide home health and/or outpatient services as needed.

Background Screening Requirement: Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
