Job summary

Discovery At Home is seeking a Physical Therapy Assistant to support care within home health and outpatient settings. You will work closely with the Physical Therapist to plan and implement patient care across resident populations.

The role emphasizes mobility, ADL assistance, patient and caregiver education, and collaborative care, with a strong focus on documentation and outcomes in Florida communities.

Qualifications

• Must graduate of an accredited Physical Therapy Assistant program.
• Must be licensed by the State as a Physical Therapy Assistant.
• One year of home health experience preferred.
• Knowledge of Medicare and Medicaid guidelines.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skills; ability to use Microsoft Windows.

Responsibilities

• Assist the Physical Therapist in planning and implementing patient care and therapy services.
• Assist patients with exercises, mobility, and ADLs as directed by the PT.
• Educate patients and caregivers on mobility, pain reduction, and injury prevention.
• Maintain accurate and timely documentation of patient progress.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Provide home health and/or outpatient services as needed to ensure outcomes.

• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed to ensure optimal outcomes

Qualifications

• Must be a graduate of an accredited Physical Therapy Assistant program
• Must be licensed by the State as a Physical Therapy Assistant
• One year of home health experience preferred
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Background Screening Requirement: Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
