Physical Therapy Assistant - Home Health & Hospice

Join CHI Home Health and Hospice to provide compassionate, personalized care in patients' homes. Our skilled staff help improve lives by caring for patients recovering from injuries, living with chronic illnesses, and other medical needs.

The physical therapist assistant provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care.

Essential Key Job Responsibilities

1. Provides direct physical therapy according to the directions of the physical therapist and in accordance with the physician's plan of care.
2. Treats patients to relieve pain, develop or restore function, and maintain maximum performance.
3. Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
4. Utilizes physical agents, including heat, cold, water, light, electricity, ultrasound, and massage.
5. Observes and reports to the physical therapist the patient’s reaction to treatment and any changes in the patient’s condition.
6. Instructs patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
7. Maintains necessary records, including clinical notes and conference notes, within 24 hours of patient care.
8. Participates in Home Care team meetings and in-service Education Program.
9. Performs other duties as assigned.

Qualifications

Graduate of an accredited physical therapist assistant program with an Associate’s degree in science or applied science approved by the American Physical Therapy Association.

PTA:MA, BLS-CPR

Must have a valid driver’s license & auto liability insurance according to company policy.
