Heritage Place Care & Rehabilitation in Tennessee is seeking a compassionate Physical Therapy Assistant to join our in-house team. You will work across skilled nursing, assisted living, and outpatient therapy, helping seniors regain quality of life and independence.
The role focuses on carrying out treatment plans under the Licensed Physical Therapist, documenting progress, and participating in care planning meetings. State licensure is required, and compensation is based on experience.

The primary purpose of this position is to plan, organize, develop, and assist in facilitating our Physical Therapy Services in accordance with applicable federal, state, and local standards, guidelines, regulations to ensure that the highest degree of quality care can be always maintained.
Pay Based on experience.
Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
Communicate with the Physical Therapist any pertinent information concerning patient progress, concerns, and plans.
Be proficient in therapeutics needs to treat patients based on their presentation and instructions given by the Physical Therapist
Participate in patient Plan of care meetings if asked by the Director of Rehab.
Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedures.
Adherence to the Required practice acts, code of conduct, ethics during professional practice as an Physical Therapy Assistant
Adherence to Company requires policy, procedures and code of conduct.
Possess an Active State Licensure or certification
