Job Summary :

Physical Therapy Assistants work under a physical therapistto help our residents with physical injuries or illness of to regain rangemovement and control pain. Under the PT's direction and plan of care, the PTAhelps residents who have lost motor function due to accidents, illness or age.

Responsibilities andDuties :

• Works under the direction of thePhysical Therapist to plan, implement, or administer programs that restore orenhance performance in individuals with functional impairments.
• AssistsPhysical Therapist with implementing treatment plans.
• Reportsto supervisors on patients' progress and discharge recommendations.
• Attends and reports accurately apatient's status at weekly UR meeting and interdisciplinary care plan meeting.
• Observes and records patients'progress, response to treatment and limitations and maintains this informationin electronic medical record.
• Completesrequired documentation at point of service.
• Identifiesneed for modification of treatment plan and reports to the Physical Therapist.
• Educatesstaff, family and caregivers as indicated.
• Identifies needs for programdevelopment and implements programs according to company policies andprocedures.
• Coordinates in-services forfacility staff as need is determined, to ensure safe and effective carry overof Restorative Programs and overall call of the residents.
• Takes apatient centered approach to provision of care.
• Carry out all job responsibilitiesin a professional manner at all times, upholding excellence in quality of care.

Qualifications :

Education : Graduate of an accredited therapy program

Licenses / Certifications : Current Physical Therapy Assistant state license
