Responsibilities

Join CHI Home Health and Hospice as a Physical Therapist Assistant and experience a fulfilling career dedicated to compassionate, personalized home care. Our skilled staff improve lives by providing care for patients recovering from injuries, living with chronic illnesses, and with other medical needs.

Essential Key Job Responsibilities

1. Provide direct physical therapy services under the supervision of a physical therapist and according to the physician's plan of care.
2. Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
3. Assist patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
4. Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage.
5. Observe and report to the physical therapist the patient’s reactions to treatment and any changes in condition.
6. Instruct patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
7. Maintain necessary clinical records within 24 hours of patient care, including notes and conference documentation.
8. Participate in Home Care team meetings and in-service education programs.
9. Perform other duties as assigned.

Qualifications

Graduate from an accredited physical therapist assistant program with an Associate’s degree in science or applied science approved by the American Physical Therapy Association.

Required certifications include BLS-CPR. ND licensure is required for PTA and AUTO roles.

Preferred Qualifications

Home health experience is preferred.
