Cornerstone Rehab, located in Kentucky, is seeking a full-time Physical Therapy Assistant to deliver care to adult and geriatric patients. You will work with a diverse team of clinical professionals and support patient outcomes through documentation, progress notes, and discharge summaries.

The role may involve travel to nearby centers as needed; a valid State PTA license and credentials are required, with SNF or acute care experience preferred.

Qualifications

• Valid State PTA License and Credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
