Physical Therapy Assistant
Job Summary

We are seeking a dedicated and compassionate Physical Therapy Assistant to join our team at Greenfield Skilled Nursing and Rehabilitation in Greenfield, Ohio. Responsibilities
  • Assist physical therapists in the development and implementation of treatment plans for our guests
  • Contribute to the provision of high-quality, patient-centered care in a variety of settings, including rehab-recovery suites and other areas of the facility
  • Conduct initial evaluations, assessments, and re-evaluations as necessary, and document findings in accordance with established policies and procedures
  • Develop and implement treatment plans, including exercise programs, training individuals, and educating guests and their families
  • Collab​ort​ with physical therapists and other members of the healthcare team to ensure effective communication and coordination of care
  • Maintain accurate and up-to-date records of patient progress and treatment outcomes
Requirements
  • High school diploma or equivalent required (***); an associate's degree in physical therapy or related field preferred
  • Minimum 0 years of experience in physical therapy or related field (***); experience working in a rehabilitation setting a plus
  • Possess a strong understanding of human anatomy, physiology, and kinesiology
  • Ability to lift, transfer, and support patients as necessary
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • Familiarity with electronic medical records and software applications a plus
