Premier Health Partners is seeking a Physical Therapist Assistant to provide patient therapies in homes or residences. Essential duties include implementing care plans, documenting services delivered, and ensuring communication within healthcare teams.
The ideal candidate will have an Associate degree from an accredited program, current Ohio licensure, and at least 1-3 years of clinical experience. Strong adaptability and computer skills are preferred.
The Physical Therapist Assistant directly provides physical therapy services to patients in their homes or residences. The assistant provides care as ordered by the physician, delegated by the Physical Therapist, follows policies/procedures and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the implementation of the plan of care as outlined by the physician and supervised by the Physical Therapist. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

Minimum Level of Education Required: Associate degree
Type of degree: Associates or Bachelors - Graduate of an accredited Physical Therapy Assistant Program.
Area of study or major: Physical Therapy Assistant
Current licensure to practice Physical Therapy Assistant in Ohio.
CPR Certification is required.
Minimum Level of Experience Required: 1 - 3 years of job‑related experience.
Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care.
Preferred experience: One year clinical experience in an acute care setting preferred. Clinical experience should have occurred within the last three years unless currently employed in home care.
