Position: Physical Therapist Assistant

Location: Indiana Fitness Works, Portland, Indiana at Persimmon Ridge

Requirements:

1. Graduate from an accredited Physical Therapist Assistant program. The program must be accredited by the American Physical Therapy Association.
2. Display evidence of interest in continuing education within the profession.

Typical Physical Demands:

1. Lift up to 10lbs overhead occasionally
2. Lift up to 50lbs from floor to knuckle occasionally
3. Lift up to 25lbs from knuckle to shoulder occasionally
4. Pivot transfer up to 150lbs occasionally
5. Carry up to 50lbs, 200ft frequently
6. Push/pull 75lbs, 100ft frequently
7. Fine hand manipulation, bilaterally continuously
8. Heavy grasp, bilaterally frequently
9. Visual acuity connected to 20/20
10. Tactile discrimination
11. Stand up to 2 hours frequently
12. Comprehensible verbal communication skills continuously
13. Drive up to 2 hours
14. Legible written communication continuously
15. Squat, stoop, or kneel up to 15 minutes occasionally
16. Sit up to 2 hours occasionally

Typical Working Conditions: Frequent exposure to communicable diseases, toxic substances, ionizing radiation, and medical preparations.

Duties: (This list may not include all of the duties assigned)

1. Conduct treatment plans as established by PT under supervision of PT.
2. Communicate with PT, rehab team members, and/or referring physician regarding patient’s treatment program.
3. Maintain weekly (objective) progress notes, following documentation guidelines.
4. Maintain accurate daily attendance records.
5. Complete daily time logs, modality sheets, Medicare logs, change slips, and any other required paperwork.
6. Supervise supportive staff in clerical and transporting duties as assigned.
7. Must follow all established policies and procedures of the company.
8. Communicate the Physical Therapist's designed plan of care to physician. The assistant will also take a telephone order from the physician to certify that plan of care or change the plan of care. The telephone order will then be co-signed by the supervising therapist.

Additional Requirements:

• Must be fully vaccinated against Covid-19, or have a Medical or Religious Exemption to be employed by Indiana Fitness Works.
• FULL TIME employees may be eligible for benefits. Waiting period will apply.
