Job summary

Southwest General Home Health seeks a qualified Physical Therapist Assistant to provide skilled services in patients' homes under the direction of a Physical Therapist. You will educate patients and families, ensure safety, and maintain thorough documentation per agency and Medicare guidelines.

This role requires PTA licensure in the state, at least 1 year of acute care PTA experience, and strong communication with patients, families, physicians, and the care team.

Qualifications

  • Current Physical Therapist Assistant license in the state of employment.
  • Minimum 1 year of PTA experience in an acute care setting (required).
  • Home care experience (preferred).

Responsibilities

  • Provide skilled PTA services in patients' homes per plan of care.
  • Perform prescribed therapy procedures and diagnostic tests.
  • Monitor patient progress and communicate with care team.
  • Educate patients and families on therapy techniques and safety.
