  • Help assess patient needs; plan, evaluate, and modify care to meet physical therapy intervention goals.
  • Collaborate with all disciplines to plan and evaluate team goals for each patient.
  • Support in setting realistic and achievable goals for patients/residents.
  • Document and record patient/resident condition and progress per state and federal regulations.
  • Educate patients and families in appropriate therapy methods.
  • Optimize patient functionality and skills by assisting in the screening of all residents.
  • Communicate patient status and needs to the patient, family, and other involved professionals.
