Job summary

Colfax Health and Rehabilitation of Cascadia is seeking a compassionate Physical Therapist Assistant (PTA) to work under a licensed Physical Therapist. You will help residents regain mobility, implement prescribed treatment plans, and support the rehabilitation team in a collaborative setting.

The role emphasizes patient-centered care, accurate documentation, and adherence to safety and infection-control standards. Six months in long-term care preferred; CPR current; PTA license required.

Responsibilities

  • Assist residents and patients with mobility, ambulation, transfers, and functional movement activities.
  • Implement treatment plans developed by a licensed Physical Therapist, including stretching, strengthening, therapeutic exercises, and physical interventions.
  • Support patient assessments by gathering health histories, conducting observations, and reporting findings to the Physical Therapist.
  • Monitor patient progress and communicate treatment responses, concerns, and recommendations to the therapy team.
  • Administer therapeutic procedures, including manual exercises and functional activities that promote recovery and independence.
  • Educate and motivate patients throughout their rehabilitation journey, encouraging participation and progress toward goals.
  • Instruct patients and families on therapeutic regimens, wellness practices, and the proper use of assistive devices such as canes, crutches, and prostheses.
  • Maintain accurate and timely documentation in patient and department records.
  • Ensure continuity of care by implementing established patient management plans.
  • Maintain a safe, clean, and compliant treatment environment while adhering to infection-control policies and procedures.
  • Support departmental productivity goals and contribute to high-quality patient outcomes.
  • Demonstrate punctuality and dependable attendance for scheduled shifts.

Qualifications

Education:

Graduate of an accredited Physical Therapy Assistant (PTA) program.

Licenses/Certification:

Current state PTA licensure.

Valid CPR certification.

Experience:

Six months experience in a long-term care environment preferred.
