Physical Therapist Assistant – Clarion Wellness & Rehabilitation

Join our team at Clarion Wellness & Rehabilitation in Clarion, IA, where we provide skilled nursing, post‑acute rehabilitation, and geriatric outpatient therapy services. We work exclusively in‑house, sharing the same goals and leadership as every local facility, focused on customer service, accountability, lifelong learning, caring for one another, intelligent risk taking, celebration, and ownership.

Responsibilities

• Utilize evidence‑based practice to support clinical interventions under the supervision of a Physical Therapist.
• Provide rehabilitative, skilled, and medically necessary treatment to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Administer a variety of procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
• Assess patients’ progress and communicate with the supervising Physical Therapist to adjust treatments, documenting all regulatory and clinically required information in the electronic documentation system.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders and related care.
• Contribute information for the patient’s discharge summary and overall care documentation.
