Job summary

Girling Health Care of NY is seeking an experienced Physical Therapist to conduct home visits in NYC. The role offers full-time or fee-for-service arrangements, with responsibilities to evaluate patients per physician plan of care and deliver therapy in the home setting.

Candidates should have a PT degree, NY licensure, CPR certification, and at least two years of relevant experience; driving preferred. Benefits include health/dental/vision, 401k, paid holidays, and PTO.

Qualifications

• Degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in the relevant state.
• Two years of PT experience; home health preferred.
• Strong verbal and written communication and organization.
• CPR certification current and maintained.
• Valid driver's license and insured vehicle preferred.

Responsibilities

• Evaluates home environment and patient needs.
• Assesses strength, mobility, gait, and ROM for rehab.
• Directs physical therapy treatment per plan of care.
• Instructs patient and family in use/maintenance of devices.
• Prioritizes PT needs and tracks progress.
• Reports patient response to physician.
• Initiates therapy program and trains others on goals.
• Trains patient in prosthetic device use when applicable.
• Identifies needs for additional home health services and refers.
• Prepares and submits progress summaries and discharge plans.
• Provides PT consultation to home caregivers.
• Delivers in-service education for staff.
• Participates in peer consultation and supervision of PTA.
• Oversees PT Assistants as required by policy.
• Instructs and supervises home health aides when PT is sole skilled service.

Education

Bachelor's or Master's in Physical Therapy

Girling Health Care of NY is looking for an experienced and reliable home care Physical Therapist to conduct home visits in NYC. The physical therapist contracted or employed through the Organization Provides physical therapy services to patients according to a written physician's plan of care. This may include but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• Evaluateshomeenvironment.
• Assesses for muscle strength, mobility, gait, ROM-potential for rehab.
• Directing physical therapy treatment.
• Instructing patients and families/caregivers in the use and care of therapeutic appliances.
• Determining priority needs for physical therapy
• Reporting to physician patient's reaction to treatment or changes in condition.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
• May train patient in the use of prosthetic device.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in discharge planning for patient.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Provides physical therapy consultation to home families/caregivers when indicated.
• Provides in-service education programs for nursing organization personnel as needed.
• Participates in peer consultation process.
• Supervises Physical Therapy Assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instructs. supervises and evaluates home health aide care every two (2) weeks.

Position Requirements:

• Possesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
• Licensedtopracticeasphysical therapist within theState,ifapplicable.
• Two years ofappropriateexperienceas aphysicaltherapist.Community/homehealthexperienceispreferred.
• Demonstrates goodverbalandwrittencommunication and organization skills.
• Possessesandmaintainscurrent CPRcertification.
• Preferred licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
