Middle Park Health seeks Physical Therapists for full-time or part-time positions in Fraser/Granby, CO, and Walden, CO. New grads welcome. Eligible full-time staff receive a $10,000 sign-on bonus and a housing stipend.

Responsibilities include evaluating and treating patients in outpatient clinics, with documentation and compliance under the Colorado Physical Therapy Act. Most full-time roles offer four 10-hour days and great mountain living.

Qualifications

• Graduate of an approved school of Physical Therapy.
• Current Colorado State Physical Therapy License.
• Current BLS certification or within 60 days from hire.

• We have 2 Full-Time Physical Therapist Positions available in our Fraser, or Granby CO outpatient clinic.
• We have a Part-Time Physical Therapist position available in our Walden, CO family practice clinic.

Reports to: Rehabilitation Services Director

Description: Responsible for providing Physical Therapy Services under the Colorado Physical Therapy State Practice Act. Examines, evaluates, assesses and treats patients in outpatient setting. Administers physical therapy interventions in an effort to restore function and prevent impairment and disability following injury, disease or musculo-skeletal dysfunction. Ensures documentation for services is provided timely and accurately.

Flexible scheduling and most full-time therapists work 4, ten hour days. Great opportunity to work and play in a mountain community.

Requirements:

• High School Diploma or GED equivalent.
  Graduate of an approved school of Physical Therapy. New Grads Welcome!
  Current Colorado State Physical Therapy License.
  Current BLS certification or within 60 days from hire.

Preferred:

• Board certified clinical specialist
• Dry needling certification
• Ability to maintain required documentation in EMR system, Epic experience preferred

Compensation: $40.00 to $64.50 per hour DOE *$10,000 Sign-On bonus for Full-Time* Housing Stipend for perm positions$175 or $350 per monthavailable for residents of Grand, Summit and Jackson County.
