Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Prescott Valley. We offer compassionate care, professional growth, and a family-like culture for clinicians making a meaningful impact on residents’ lives.

Qualifications

• Graduated from an accredited university with a Doctor of Physical Therapy or equivalent.
• Valid (or in process) license in the appropriate state.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients with physical functioning disorders.
• Provides skilled therapy services to patients needing rehabilitation services.
• Provides consultation and counseling to patients and families related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with licensure requirements.

Experience/Qualifications:
