Job summary

UPMC Home Health is seeking a licensed Physical Therapist to serve West Mifflin, Duquesne and surrounding communities. You will provide age-appropriate physical therapy in patients' homes, including evaluation, treatment, and patient/family education to maximize function and independence.

You will perform admission visits, develop plans of care with the physician, document thoroughly, supervise the Physical Therapy Assistant, and participate in on-call coverage.

Qualifications

  • Graduate from an approved physical therapy curriculum.
  • Minimum one year of clinical experience to practice without supervision.
  • Current licensure/registration as a Physical Therapist (PT) per state requirements.

Responsibilities

  • Provide age-sensitive PT care in patients' homes, including evaluation and treatment.
  • Maintain timely documentation including visits and orders, in line with agency standards.
  • Collaborate with physicians and family to establish plan of care.
  • Supervise Physical Therapy Assistant as directed by State Board guidelines.
  • Participate in on-call coverage rotation.

Job description

UPMC Home Health is hiring a Physical Therapist for the West Mifflin, Duquesne, and surrounding areas. Provide age-sensitive professional physical therapy care in the home setting, including admission visits, evaluation, instruction, and treatment to patients to maximize functional abilities and ensure optimal care.

Responsibilities
  • Perform in accordance with system-wide competencies/behaviors. Perform other duties as assigned.
  • Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures.
  • Focus on customer service and continually strive to perform the duties of their job in a manner that will result in optimal satisfaction.
  • Identify, develop, and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Perform duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC Home Healthcare towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Continually develop professional skills by assuming responsibility for ongoing continuing education.
  • Participate in on-call coverage rotation for the agency.
  • Assign and supervise the Physical Therapy Assistant according to the guidelines of the State Board of Physical Therapy.
  • Advise and consult with the family, inter-intradisciplinary team members, physicians, and other professionals, as appropriate, to ensure optimal patient care.
  • Sync (transmit) data the same day.
  • Maintain and complete all documentation promptly and accurately, including visit assessment, OASIS, 485, and verbal orders on the laptop device or in written documentation while in the patient's home and within the agency and regulatory standards.
  • Establish the plan of care in collaboration with the physician, patient, and UM professional in accordance with the physician's orders and patient needs and follow the admission process.
  • Evaluate, plan, and treat patients in an effective physical therapy program utilizing diagnostic, muscle, nerve, joint, and functional ability testing within the standards of Physical Therapy Practice. Make recommendations and order necessary DME when indicated.
Qualifications

Graduate from a physical therapy curriculum approved by:

  • The American Physical Therapy Association; or the Committee on Allied Health Education and Accreditation of the American Medical Association; or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association; or prior to January 1, 1996: be admitted to membership by the American Physical Therapy Association; or be admitted to registration by the American Registry of Physical Therapists; or have graduated from a physical therapy curriculum in a 4‑year college or university approved by a state department of education; or have 2 years of appropriate experience as a physical therapist and achieve a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service; or (such determinations of proficiency do not apply with respect to persons initially licensed by a state or seeking qualification as a physical therapist after December 31, 1977); or be licensed or registered prior to January 1, 1966, and prior to January 1, 1970, had 15 years of full‑time experience; or if trained outside the United States, graduated since 1928 from a physical therapy curriculum approved in the country in which there is a member organization of the World Confederation for Physical Therapy; or meet the requirements for membership in a member organization of the World Confederation for Physical Therapy.
Minimum one (1) year of clinical experience to practice without supervision, or less than one (1) year if practicing under supervision (i.e., ALF/PCH). May, in the course of their job duties, be in rooms where medications are stored. No contact with medications is allowed.
Licensure, Certifications, and Clearances

Licensure/Certification: Basic Life Support or Cardiopulmonary Resuscitation (CPR) required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
