Job summary

The Physical Therapist (PT) at The Willows at Okemos in Okemos, MI is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment.

Must hold a current state PT license or certification; CPR certification preferred. 0–1 years of experience expected, with education from an accredited Physical Therapy program. Strong emphasis on documentation and collaboration within the care team.

Qualifications

• Must be licensed or certified as a Physical Therapist in the state.
• CPR certification preferred.
• 0–1 years of experience in physical therapy.

Responsibilities

• Conduct thorough patient assessments to determine rehabilitation needs.
• Develop and implement individualized treatment plans including therapeutic exercises and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate documentation of progress and treatment plans in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
