Job summary

Trilogy Health Services, LLC in Michigan is seeking a Physical Therapist to provide a full range of PT services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary setting at The Willows at Okemos.

The position requires a current PT license in the state, CPR certification (preferred), and 0–1 years of experience. You will develop personalized treatment plans, educate patients and families, and maintain compliant documentation.

Qualifications

• Degree in Physical Therapy from an accredited program.
• 0-1 years of experience.
• Must have and maintain a current, valid state PT licensure.
• Current CPR certification, preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, and body mechanics.
• Maintain accurate documentation of patient progress and outcomes per facility policies.
