Job summary

Community Health Systems is seeking a Physical Therapist for PRN weekend coverage at Houghton and Sahuarita campuses. Qualifications

Responsibilities

• Conduct thorough PT evaluations to assess patient needs and develop individualized treatment plans.
• Administer safe and effective therapy treatments and document interventions.
• Reassess and adjust treatment plans within appropriate timeframes based on patient progress.
• Provide clear, concise documentation of evaluations, treatment plans, progress notes, and billing information.
• Educate patients and families on treatment plans, exercises, and post-treatment care to support recovery.
• Operate therapy equipment safely and maintain a clean, organized, and safe environment for patients and staff.
• Collaborate with referring physicians and interdisciplinary team members to ensure continuity of care.

Job Summary

The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.

Schedule: PRN position for weekend coverage at both Houghton and Sahuarita campuses

Qualifications

• Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
• Maintains compliance with facility policies, including safety, infection control, and risk management procedures.
• Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
• Performs other duties as assigned.
