Job summary

A healthcare organization located in Kingman, Arizona, is seeking a Physical Therapist to join their Therapy Services team. This role includes conducting evaluations, developing treatment plans, and providing patient care. The ideal candidate will hold a four-year degree in Physical Therapy, a current state license, and recent experience with adult and geriatric populations. Qualifications

  • Graduate of an APTA approved program.
  • Current PT License through the AZ Board of Physical Therapy.
  • BLS certification through American Heart Association or Red Cross.
  • Recent experience with adult and geriatric population.

Responsibilities

  • Conduct a Physical Therapy evaluation or screen.
  • Develop an individualized Plan of Care with functional goals.
  • Provide appropriate therapeutic interventions.
  • Document the course of patient care and modify plan as necessary.

Position Purpose

Key Responsibilities
  • Conduct a Physical Therapy evaluation or screen.
  • Develop an individualized Plan of Care with functional goals.
  • Provide appropriate therapeutic interventions.
  • Evaluate patient response to treatment and modify plan as necessary.
  • Document the course of patient care including progress made and the continuing need for treatment.
  • Provide patient and/or family education to help achieve goals.
  • Communicates and demonstrates a professional image for patients, families, clients, coworkers, and others.
  • Adhere to policies, rules, and regulations from KHI, Arizona State Board of Physical Therapy, and Medicare.
  • Assists with orientation of new staff as requested.
  • Meets KHI requirements for skills competency and safety.
  • Participate in continuing education/professional development activities.
  • Assists with the Physical Therapy and Physical Therapy Assistant student program.
  • Participate in staff meetings, client relation and marketing activities.
  • Documentation is complete and accurate, meeting all KHI, Arizona State Board of Physical Therapy, and Medicare regulatory requirements.
  • Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements, and is justified in the documentation.
  • Individual productivity meets department standards.
  • Patient satisfaction will meet or exceed department standards.
Qualifications
  • Education: Graduate of an APTA approved four year bachelor’s degree program, or foreign equivalent.
  • Licensure: Current PT License through the AZ Board of Physical Therapy
  • Certification: BLS through the American Heart Association or American Red Cross
  • Experience: Recent experience working with an adult and geriatric population
