Position Purpose
All KHI employees are expected to perform their respective tasks and duties in such a way that supports KHI’s vision to be among the kindest, highest quality health systems in the country.
Key Responsibilities
- Conduct a Physical Therapy evaluation or screen.
- Develop an individualized Plan of Care with functional goals.
- Provide appropriate therapeutic interventions.
- Evaluate patient response to treatment and modify plan as necessary.
- Document the course of patient care including progress made and the continuing need for treatment.
- Provide patient and/or family education to help achieve goals.
- Communicates and demonstrates a professional image for patients, families, clients, coworkers, and others.
- Adhere to policies, rules, and regulations from KHI, Arizona State Board of Physical Therapy, and Medicare.
- Assists with orientation of new staff as requested.
- Meets KHI requirements for skills competency and safety.
- Participate in continuing education/professional development activities.
- Assists with the Physical Therapy and Physical Therapy Assistant student program.
- Participate in staff meetings, client relation and marketing activities.
- Documentation is complete and accurate, meeting all KHI, Arizona State Board of Physical Therapy, and Medicare regulatory requirements.
- Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements, and is justified in the documentation.
- Individual productivity meets department standards.
- Patient satisfaction will meet or exceed department standards.
Qualifications
- Education: Graduate of an APTA approved four year bachelor’s degree program, or foreign equivalent.
- Licensure: Current PT License through the AZ Board of Physical Therapy
- Certification: BLS through the American Heart Association or American Red Cross
- Experience: Recent experience working with an adult and geriatric population
