Job summary

BAYADA Home Health Care in Hawaii is seeking a Physical Therapist to join our Oahu Visits team. You will assess clients in their homes, design individualized treatment plans, and apply evidence-based techniques to help clients reach their goals while remaining at home.

You will work with a multidisciplinary team, document care using EMR, and participate in training and ongoing professional development. We offer flexible schedules, comprehensive health benefits, and a mission-driven work

Qualifications

• Current Hawaii Physical Therapist license.
• Graduate of an APTA-approved PT program or equivalent accreditation.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Perform diagnostic tests and measurements for mobility, strength, balance, and safety.
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions.
• Apply wound care and protective interventions with evidence-based dressings and topical agents.
• Continuously assess and revise the PT care plan.
• Educate clients and caregivers to promote health, safety, and independence.
• Accurately document observations and interventions using a touch pad tablet.

BAYADA Home Health Care is currently seeking a Physical Therapist (PT) to join our Oahu Visits team. PT’s work with clients in their home, craft treatment plans, and apply innovative techniques to help them reach their goals. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact:

• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance, balance testing, and safety assessments
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents
• Continually assess and revise the Physical Therapy - PT care plan
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a BAYADA Physical Therapist:

• A current Physical Therapist - PT license in Hawaii
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
• Solid computer skills; prior experience with electronic medical records (EMR) preferred

Type: Full-Time or Part-Time

Base Pay, depending upon qualifications and subject to negotiation: $70-$80 per point
