Job summary

Jackson Therapy Partners seeks a traveling Physical Therapist to help patients recover from injuries and manage chronic conditions in Santa Rosa, California. You will handle a geriatric caseload, visit up to six patients daily, and collaborate with the care team.

Requirements include a PT degree, active California license, and at least one year of skilled home health experience. Monday–Friday schedule with competitive benefits and travel housing support offered.

• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• CA license required
• BCLS required

• 1 year of recent skilled home health experience within the last 3 years required
• OASIS experience required for start of care
• Own transportation required
