Job summary

Jackson Therapy Partners, LLC is seeking a traveling Physical Therapist to help patients recover from injuries and manage chronic conditions across partner facilities in Connecticut. This role offers flexible assignments, dayshift 8:30-16:30, and the chance to experience life in new communities while delivering high-quality rehab care.

You hold a PT degree from an APTA-approved program, a Connecticut license, and BLS certification.

Qualifications

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Connecticut Physical Therapist license required to start the assignment.
• Experience in Skilled Nursing Facility or long-term care setting preferred.

Responsibilities

• Provide physical therapy services in travel assignments across partner facilities.
• Manage caseload in a fast-paced rehabilitation environment and document progress in EMR.
• Collaborate with interdisciplinary teams to plan and implement mobility and rehab plans.

• Demonstrated proficiency with patient mobility training, fall prevention programs, and documentation in an electronic medical record preferred.
• Ability to manage a caseload in a fast-paced rehabilitation environment and collaborate with interdisciplinary teams.
