Job summary

Jackson Therapy Partners, LLC seeks a traveling Physical Therapist to support a Skilled Nursing Facility in Colchester, CT. You will assess and treat patients, develop mobility plans, and collaborate with an interdisciplinary team.

Qualifications

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Connecticut Physical Therapist license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross may be required.
• Experience in Skilled Nursing Facility or long-term care preferred.
• Proficiency with mobility training, fall prevention, and EMR documentation.
• Ability to manage a caseload and collaborate with interdisciplinary teams.

Responsibilities

• Provide physical therapy services to SNF residents and acute patients as assigned.
• Develop and implement mobility and rehabilitation plans.
• Document assessments and progress in the EMR system.
• Collaborate with physicians, nurses, and other therapists to optimize patient outcomes.
