Job summary

Jackson Therapy Partners is seeking a traveling Physical Therapist to help patients recover from injuries and manage chronic conditions while moving to new locations for assignments.

Requirements include a PT degree from an APTA-approved program, active state licensure, and Home Health experience. The position uses Home Care Home Base EMR, with travel within an hour radius of the hub city and Teams/Video Call interviews.

• BLS Certification May Be Required from AHA or ARC
• Home Health experience required

Responsibilities

• Provide PT services to patients in home health settings and help them recover from injuries or manage chronic conditions.
• Work with the care team to coordinate treatment plans and meet productivity expectations of 30 points per week.
• Utilize Home Care Home Base EMR for documentation and scheduling.
• Travel within an hour radius of the hub city based on referral volume; interview via Teams/Video Call.
