Job summary

The PT will work with an interdisciplinary healthcare team to develop plans, monitor progress, and support recovery in home settings, using EPIC for documentation and coordination, and may supervise PT

Qualifications

• EPIC experience required.
• DPT degree required for applicants licensed on or after January 2016, or an accepted foreign equivalent.

Responsibilities

• Conduct patient evaluations to assess physical functioning and identify areas requiring therapeutic intervention.
• Develop individualized treatment plans based on patient needs and clinical assessments.
• Provide physical therapy interventions using appropriate clinical techniques and treatment protocols.
• Establish individualized care and discharge plans and communicate them to the appropriate healthcare team members.
• Complete accurate and comprehensive clinical documentation within established deadlines.
• Use EPIC for clinical documentation and patient care coordination.
• Supervise assigned physical therapy assistants as required.
• Collaborate with the interdisciplinary healthcare team to support patient recovery and independence.
• Perform other job-related duties as assigned.

Job Requirements

Education

Required Licenses and Certifications

• Valid Physical Therapist (PT) license in the applicable state of practice.
• Current CPR or Basic Life Support (BLS, HS-BLS, or RQI BLS) certification from an approved vendor.
