Job summary

Centra Healthcare Solutions Inc seeks an experienced or newly graduated Physical Therapist for a contract assignment at an outpatient clinic. Shift: Monday–Friday, 8/9 AM–5/6 PM.

The PT will develop plans of care, provide skilled PT services per physician orders, and collaborate with the care team to achieve patient goals. Requirements include graduation from an accredited program, current PT licensure (or in process), and CPR certification.

Qualifications

• Must have graduated from an accredited school.
• Current state licensure as a Registered Physical Therapist or in the licensure process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services in accordance with physician orders.
• Assess, plan, evaluate and modify care to meet goals.

Requirements include:
