Position Overview

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist (PT) to join their growing team.

The Physical Therapist is responsible for assessing referred patients that require rehabilitation services, providing direct patient care to evaluate medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation. The PT will establish and administer a treatment program with specific goals determined by the patient's capacity and tolerance under the direction of the Physician.

Key tasks include setting realistic and achievable goals, documenting and recording patient condition, educating patients and families in appropriate physical therapy methods, and providing skilled PT services/interventions in accordance with physician orders.

Responsibilities
  • Assess referred patients that require rehabilitation services.
  • Provide direct patient care to assess medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
  • Establish and administer a treatment program with specific goals according to the patient's capacity and tolerance.
  • Set realistic and achievable goals for the patient.
  • Document and record the patient’s condition.
  • Educate patients and families in appropriate physical therapy methods.
  • Provide skilled physical therapy services/interventions per physician orders.
  • Assess patient needs, plan, evaluate and modify care to meet PT goals.
  • Collaborate with all disciplines to plan and evaluate team goals for each patient.
