Responsibilities

• Assess referred patients requiring rehabilitation services and provide direct patient care, evaluating medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.
• Establish and administer a treatment program with specific goals tailored to each patient’s capacity and tolerance, under the direction of the physician.
• Set realistic and achievable goals, document and record patient condition, and educate patients and families on appropriate physical therapy methods.

Requirements

• Graduated from an accredited physical therapy program.
• Current state license as a Registered Physical Therapist (or actively pursuing licensure).
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate, and modify care to meet physical therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
