Blue Palms Health & Rehabilitation Center in Deland, FL is seeking a licensed Physical Therapist to join our subacute rehabilitation team. The role focuses on creating customized treatment plans, assessing and educating patients, and collaborating with the care team to deliver patient-centered services.

Formación

• Licensed Physical Therapist in Florida
• Strong communication, problem-solving, and clinical skills
• Ability to work independently while being a team player
• Prior experience in skilled nursing or subacute settings is a plus
• For per diem applicants: ability to work 10–15 hours per week consistently

Responsabilidades

• Create customized treatment plans that help patients regain mobility, strength, and confidence.
• Use your expertise to assess, treat, and educate patients and families on recovery strategies.
• Collaborate with nurses, occupational therapists, and physicians to provide top-notch, patient-centered care.
• Track patient progress and celebrate improvements—every step forward counts!
• Maintain compliance with all facility policies and regulatory standards.
