PHYSICAL THERAPIST – SOUTH MOUNTAIN POST ACUTE – PHOENIX, AZ

South Mountain Post Acute Care in Phoenix, AZ, is seeking a FULL TIME PT to join our in-house therapy team. We offer excellent leadership, facility-level decision making, competitive compensation, and a supportive work environment where your efforts are appreciated.

Experience/Qualifications:
  • Licensed Physical Therapist
  • Open to PTs at all experience levels
Key Responsibilities:
  • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders, developing appropriate care plans following all standards.
  • Use standardized assessments and evidence-based practices to support interventions.
  • Provide rehabilitative, skilled, and medically necessary treatments.
  • Administer procedures such as ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
  • Assess patient progress and modify treatments accordingly, documenting in clinical records.
  • Evaluate treatment outcomes and adjust services; refer patients as needed.
  • Offer consultation and counseling to patients, families, and caregivers.
  • Communicate effectively with team members regarding patient progress and treatment plans.
  • Prepare comprehensive discharge summaries per regulatory standards.
