Job Description
The Physical Therapist reflects our mission, vision, and values, adhering to our Code of Ethics, Corporate Compliance Program, and all relevant policies and standards.
- Licensed physical therapist complying with the Code of Ethics and Standards of Practice as per the Illinois State requirements.
- Provides direct patient care, participates in teaching, committee work, and departmental process improvements.
- Serves as a role model for all therapists and interns in professional, clinical, communication, education, and leadership activities.
Responsibilities

- Utilizes resources efficiently and manages work schedules to complete tasks timely.
- Contributes to continuous improvement initiatives and departmental growth.
- Adapts treatment plans creatively to meet individual patient needs, demonstrating competency in evaluation and treatment modalities.
- Assumes full responsibility for patient management from initial contact to discharge, maintaining primary responsibility over care plans.
- Documents all notes accurately in the electronic medical record, reflecting patient responses and treatment outcomes.
- Provides educational materials suited to patient understanding and motivation.
- Participates in departmental committees, projects, and educational efforts, including mentorship and student programs.
- Builds professional relationships with healthcare team members and participates in marketing efforts.
- Fosters respectful communication and accepts constructive feedback for continuous improvement.
- Completes non-patient care responsibilities proactively and within established timelines.
- Maintains productivity standards and demonstrates flexibility in schedule and assignments to meet patient care needs.
Qualifications
- Required: Bachelor’s Degree in Physical Therapy, Illinois State license, BLS certification from the American Heart Association.
- Preferred: Master’s or Doctoral Degree in Physical Therapy.
