Job summary

PruittHealth is seeking a licensed Physical Therapist (PT) for a PRN role in Toomsboro, GA. The position involves evaluating and treating patients across orthopedic, neurological, and mobility-related conditions in a post-acute care setting.

You will develop individualized plans, monitor progress, document outcomes, and educate patients and families. Experience in skilled nursing or home health is preferred; new grads may apply where eligible.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the state of practice.
• New graduates welcome where applicable.
• Experience in skilled nursing, rehabilitation, home health, or post-acute care preferred.

Responsibilities

• Evaluate patients with orthopedic, neurological, and mobility-related conditions.
• Develop and implement individualized treatment plans.
• Improve strength, balance, gait, endurance, and overall functional mobility.
• Monitor patient progress and modify interventions as needed.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, nurses, therapists, and caregivers.
• Educate patients and families on rehabilitation goals and home exercise programs.
