PruittHealth is seeking a licensed Physical Therapist (PT) for SNF care at a facility located at 112 Hospital Drive, Washington, GA 30673. The role supports patients in mobility restoration and independence, with compensation up to $130,000 annually and a sign-on bonus up to $8,000.

Schedule can be FT, PT, or PRN with comprehensive benefits and growth opportunities. Eligible new graduates are welcome; prior SNF/post-acute care experience is preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the state of practice.
• New graduates welcome where applicable.
• Experience in skilled nursing, rehabilitation, home health, or post-acute care preferred.
• Strong clinical, communication, and documentation skills.

Responsabilités

• Evaluate patients with orthopedic, neurological, and mobility-related conditions.
• Develop and implement individualized treatment plans.
• Improve strength, balance, gait, endurance, and overall functional mobility.
• Monitor patient progress and modify interventions as needed.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, nurses, therapists, and caregivers.
• Educate patients and families on rehabilitation goals and home exercise programs.
