You will provide high-quality care, collaborate with facility staff, supervise PTAs, and maintain accurate documentation to track progress and outcomes. This role emphasizes patient education and measurable goals in a supportive environment.

Responsibilities

• Provide high-quality patient care in SNF settings.
• Evaluate new patients and develop plans of care with measurable functional goals.
• Treat patients and educate them about injury, progress, and expected outcomes.
• Supervise Physical Therapist Assistants and collaborate with facility staff.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes.
