Job summary

PruittHealth is seeking a licensed Physical Therapist (PT) to join our Franklin SNF team. You will evaluate patients, develop and implement treatment plans, and help restore mobility and independence in a post-acute care setting.

This PRN role offers competitive compensation up to $130,000 annually, based on experience, schedule, and location, plus benefits, growth opportunities, and a collaborative team focused on clinical excellence.

Qualifications

• Licensed Physical Therapist in the state of practice.
• Experience in skilled nursing or post-acute care is preferred.
• Strong communication and documentation skills.

Responsibilities

• Evaluate patients with orthopedic, neurological, and mobility-related conditions.
• Develop and implement individualized treatment plans.
• Improve strength, balance, gait, endurance and functional mobility.
• Monitor progress and modify interventions as needed.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, nurses, therapists, and caregivers.
• Educate patients and families on rehabilitation goals and home exercise programs.
