Step Up Rehab, LLC, located at Northbank Center for Rehabilitation and Healing in Jacksonville, FL, seeks a Physical Therapist (PT) for a full-time SNF role with a split assignment between Northbank and Lakeside centers. You will help residents regain function and independence through skilled therapy and collaborative care.

The position requires Florida licensure (or eligibility) and a PT degree from an accredited program.

Qualifications

• Graduate of an accredited PT program.
• Active Florida PT license (or license-eligible).
• SNF or post-acute experience preferred.
• Net Health knowledge preferred.

Responsibilities

• Provide skilled PT therapy services to residents in a SNF.
• Collaborate with the interdisciplinary team to improve safety, function and quality of life.
