Start Date: ASAP | Available Shifts: 8 D | Pay Rate: $1463.00 ‐ $1685.00

Position Details:

• PT‐ SNF needed ASAP for 13 weeks
• Illinois licensure required
• BLS certification required
• At least 1 year SNF experience

Required Qualifications:

• Physical Therapist, Skilled
• Experience: 1 year
• Location: IL
• Certifications: BLS (Copy needed)
• SSN required
• Date of Birth required
• Work History: No more than 30 days of gap in the last year
• References: 1 reference in entire work history
