Job summary

Avante at Ocala is seeking a Physical Therapist to oversee and coordinate resident assessments, ensuring compliance with federal, state, and local regulations for our in-house therapy department. This role focuses on delivering high-quality physical therapy services to residents in a skilled nursing setting.

The ideal candidate has a BS in Physical Therapy, is a registered PT or eligible for licensure, and has 0–3 years of long-term care experience.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• 0-3 years of long-term care experience.

Responsibilities

• Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with department procedures.
• Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
• Evaluates residents to obtain data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Adheres to established confidentiality standards.
• Implements and conducts treatment as outlined in treatment plan.
• Follows management direction.
• Performs other duties as assigned.
