Job summary

Physical Therapist position at a Skilled Nursing Facility in Concord, NH. You will assess and treat elderly patients recovering from surgery or illness, develop individualized plans, guide them toward greater independence, and document progress accurately.

You will collaborate with nurses, PT assistants, and other clinicians in a supportive team, provide education to patients and families, and uphold safety and regulatory standards while delivering compassionate, patient-centered care.

Qualifications

• Active state license as a Physical Therapist in California.
• Current BLS/CPR certification.
• Minimum of 1 year of recent PT experience, ideally in a SNF or geriatric setting.
• Strong clinical reasoning and leadership skills.
