Job summary

Height Street Skilled Center in Bakersfield, CA, is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility. You will evaluate patients, develop individualized plans, and collaborate with a multidisciplinary team to restore mobility and independence.

The role requires a CA license and CPR certification, with SNF experience preferred. A supportive, patient-centered environment offers opportunities for growth and ongoing education.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid California Physical Therapist license.
• CPR certification (current and valid).
• Experience in a skilled nursing or long-term care setting preferred, but not required.
• Strong communication and documentation skills.
• Ability to motivate and encourage patients with compassion.

Responsibilities

• Evaluate and assess patients to develop individualized treatment plans.
• Provide physical therapy services to patients recovering from injury, illness, or surgery.
• Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients, families, and caregivers on therapy techniques and home exercises.
• Maintain accurate and timely documentation in accordance with state, federal, and facility guidelines.
• Ensure compliance with all regulatory and facility standards.
• Contribute to team meetings and care planning for residents.

We are seeking a dedicated and patient-focused Physical Therapist (PT) to join our rehabilitation team at our Skilled Nursing Facility. As a Physical Therapist, you will play a critical role in helping our residents improve mobility, reduce pain, and regain independence after illness, injury, or surgery. You’ll be part of a multidisciplinary team committed to providing the highest standard of care in a supportive and collaborative environment.
