Job summary

Uintah Basin Rehabilitation and Senior Villa in Roosevelt, UT is seeking a Physical Therapist for a full-time role with a sign-on bonus and relocation assistance. You will evaluate residents, develop treatment plans, and provide therapy to improve mobility.

Collaborate with an interdisciplinary team, educate residents and families, and maintain accurate documentation while delivering compassionate care in a smaller, community-focused setting.

Qualifications

• Current Physical Therapist license in Utah
• Graduate of an accredited Physical Therapy program
• Strong clinical and assessment skills
• Strong communication and interpersonal skills
• Ability to work effectively within an interdisciplinary team
• Ability to maintain accurate documentation and meet regulatory requirements

Responsibilities

• Evaluate residents and participate in therapy screenings
• Develop and implement individualized treatment plans
• Provide physical therapy services to improve mobility and function
• Collaborate with the interdisciplinary care team to achieve resident goals
• Educate residents and families on treatment plans and progress
• Maintain timely and accurate clinical documentation
