Confluent Health in Hayward, Wisconsin is seeking a licensed Physical Therapist to join our patient-centered care team. You will evaluate patients, create individualized plans of care, and deliver hands-on therapy in a supportive, mentorship-focused environment.

We offer a $15,000 bonus option, student loan repayment, a 401(k) with 4% match, generous PTO, medical/dental/vision, disability, and opportunities for certification and leadership development through our Evidence in Motion partnerships.

Qualifications

• Licensed Physical Therapist (or ability to obtain licensure in Wisconsin)
